Cancellation Policy
If you need to cancel or reschedule an appointment, please provide one working day's notice. If you fail to provide the requested notification for an appointment, you may be assessed a cancellation fee.
Co-pay Policies
Co-pays are due at the time of service, a $10.00 statement processing fee will be charged for those not paid at the time of service.
